Player Registration Form
Players must register once a year. Our enrollment period begins September 1. We would like to encourage you to register online because it keeps our expenses down. If you do not have a credit card or Pay Pal account, you can download a form here.

Team Registration Form
All teams must register online. Please see the Team Registration page.

Permission to Play Adult League
This form is needed for youth who play with a youth club such as MSC or CSC and play in the adult league. This must be turned in one week before your first game or there will be a penalty of $25.
